Our Process
Identification and screening
Primary care providers use lifestyle assessment tools during annual wellness visits to evaluate nutrition, movement, sleep, stress, social connection, and substance use.
Provider order or Self-Referral
The primary care physician submits an electronic medical record (EMR/EPIC) referral order to an integrated or standalone lifestyle medicine clinic, though some centers also accept direct patient self-referrals.
Onboarding and assessment
A program coordinator or nurse contacts the patient for initial onboarding, followed by a comprehensive clinical assessment with a lifestyle medicine physician or nurse practitioner.
Care Team Integration
Patients are routed into APEX’s individualized or shared medical appointments (group visits) alongside APEX’s dietitians and certified coaches for targeted behavior modification.
