APEX staff initiate the process of translating a physician's general medical exercise clearance into a specific, individualized workout plan detailing exact movements, sets, repetitions, and heart rate zones. Specialized training sessions tailored specifically to safely manage symptoms, improve function, and lower risks associated with conditions like type 2 diabetes, hypertension, and obesity.

Patient safety comes first. That’s why APEX’s EIM programs ask for medical clearance from a healthcare provider. Referral forms are conveniently available on our website to help make the process easy. Simply print out the referral form and scan it to us.

Insurance billing is currently available only for the APEX Wellness Hub’s Healthy Lifestyle Program, with Medicare coverage for those who qualify. All other programs and services are self-pay.
